Stepparent Letter Of Intent To Biological Parent

I would like to let you know of my intention to adopt {Child Name} and be recognized as {his/her} legal {mother/father}. Since I married {Spouse}, {he/she} and I have been the primary caregivers of {Child Name}, and we have become the family I have always imagined building.

{Child Name} has expressed {his/her} wish that I make our parent/child relationship formal, and I am writing to you to request that you consent to a termination of your parental rights. Should you consent to this, the following changes would apply:

• You would no longer be legally obligated to pay child support.
• You would lose the legal right to visit or host {Child Name}.
• You would lose the right to medical or financial decisions made on {Child Name}'s behalf.

I have attached a consent form and encourage you to consider it. Please let me know when you have a decision.
